• Part time 18.75 hours per week, temporary up to 30 Jun 2021 - Total Indicative Remuneration $92,017 - $106,446 (Pro-rata) - AHP2

The Social Worker, CALHN Intermediate Care primary role is to provide discipline specific clinical services within a team environment using an individualised assessment and service delivery approach for people with a complex care needs who frequently present to hospital and are at high risk of readmissions. The role includes, development, coordination, implementing and evaluating intermediate and primary health social work services by:

• Undertaking all phases of intervention processes and/or projects in the social work area, including direct client work.
• Selecting, adapting and developing new and innovative methods within the scope and objectives of the social work service and accepting professional responsibility for the quality and effectiveness of activities; provide professional advice and/or supervision and consultancy to management, other staff and agencies.
• Supporting the Integrated care rapid assessment team
