ACE ED Social Work Care Coordinator Grade 2


This is a temporary full time position until January 2022 (maternity leave replacement).


The ACE (Advice, Coordination & Expertise) ED Team is a multidisciplinary team providing care coordination services to clients presenting to the Emergency Department. The ACE ED Care Coordinators actively screen all patients presenting to the Emergency Department at Footscray, Sunshine and Williamstown Campuses and prioritise assessments to improve discharge outcomes. The service aims to improve health outcomes for the frail aged, complex and socially disadvantaged patients with complex psycho-social needs. The service also provides rapid identification, assessment and care coordination to facilitate the patient journey through the Emergency Department and wider hospital, with the aim of reducing unplanned admissions. In addition, patients can be supported post discharge from ED through our Rapid Allied Health@ home service. The ED service operates between 7:30am and 9:00pm, 7 days a week and we are seeking an experienced social worker to join the team.


Assessments are holistic in nature with the aim of identifying issues that will affect the patient’s safe discharge from ED. Clinicians work within a transdisciplinary framework (supported by comprehensive competency training) with areas of assessment and intervention including but not limited to:



  • Mobility

  • Activities of Daily Living

  • Home set up

  • Social Profile 

  • Psychosocial needs


After thorough assessment, the ACE ED Clinicians will develop a plan for safe discharge of the patient home. Some interventions that may be implemented can include:



  • Provision of equipment to improve safety at home 

  • Referral to a community/local government service for assistance

  • Referral to a Western Health community based service

  • Urgent home visit (for patients in a private dwelling or supported accommodation)

  • Urgent In-reach visit (for patients from a Residential Aged Care Facility)


This team also aims to support patients frequently presenting to Western Health Emergency Departments via the development of proactive strategies such as ED individual patient plans of care.
