St Andrews War Memorial Hospital is currently inviting applications from experienced Registered or Clinical Nurses to join their team in a Permanent, Part-Time Complex Care Coordinator position.

About the Role:

The Clinical Care Coordinator provides assistance to the departments and wards by facilitating a seamless end to end experience for our patients and their care. This role will ensure discharge planning is undertaken in a timely manner to optimise patient outcomes and length of stay. You will liaise with a range of internal stakeholders to coordinate patient discharge plans, patient follow-ups after discharge and readmissions reviews when required.

The successful applicant will ideally present with:

  • Bachelor of Nursing or equivalent with current AHPRA Registration
  • Minimum 5 years’ experience in an acute care setting with recent experience of Complex Care Management
  • Commitment to the delivery of high-quality patient care and customer service
  • Ability to work supportively and maintain effective working relationships within a multidisciplinary team
  • Demonstrated commitment to professional development and quality improvement
  • Demonstrated problem solving and conflict resolution skills

This is a permanent part-time position, working a base roster of 24 hours per week.
