  • This is a full time role, however we can consider 4 days per week for an experienced Community Palliative Care Nurse, Monday to Friday 7am-4.30pm
  • Role based in Neringah, however occasional travel across all three sites may be required.

About the role

Reporting into the Nurse Manager and Team Leader, The Clinical Nurse Specialist works within the Mission and Mission in Action of HammondCare to provide expert clinical support of the activities of the Palliative Care Team. The CNS is an expert in palliative care with aged care assessment skills and knowledge. Additionally the CNS is to provide support of the educational and professional development of nursing groups, palliative care staff, patients, carers and their families. A core function of the role is to be a conduit for residents of aged care facilities or other hospitals transitioning to end of life care and establishing links with appropriate clinical services within HammondCare. The CNS is able to extend nursing practice by facilitating timely patient review and manage patients that present with palliative/end of life care needs.

Key Focus Areas:

  • Provide clinical leadership and promote HammondCare’s Mission and Mission in Action.
  • Provide specialist clinical care in specialty service area.
  • Participate in daily team discussions and manage allocated caseload.

About you

  • Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action.
  • Registered Authorised RN with AHPR that has completed the tertiary requirements including a degree as Registered Nurse.
  • Ability to independently assess, investigate and implement appropriate treatment for patients within advanced nursing practice guidelines.
  • Demonstrated clinical competency in advanced practice management in Supportive and Palliative Care and specific interest in aged care.
  • Demonstrated ability to work autonomously and have effective communication and negotiation skills.
  • Demonstrated ability to work independently within the wider multidisciplinary team.
  • Demonstrated effective communication and interpersonal skills (written and verbal)
  • Demonstrated organisation, networking and time management skills
  • Demonstrated ability to work with sensitivity, empathy, patience and flexibility.
  • A current drivers license and safe driving record.
  • Willingness to travel between HammondCare services and sites.
