About the opportunity

We’re currently recruiting for multiple full-time permanent Care Coordinators to join our team based in Clayton South, commencing March 2021. As the newest member to our team, you’ll be responsible for the coordination of staff (including identifying and preparing appropriately skilled staff in line with client requirements), rostering and ensuring coverage of care for our clients.

A day in the life as a Care Coordinator with CareChoice, may see you supporting our Participants across variety of shifts from Monday to Friday including morning, day, afternoon and overnight shifts with:

  • Managing the coordination of our Support Workers to deliver disability services in line with Client Care requirements and preferences
  • Prioritizing and managing a high volume of inbound and outbound calls and adhoc administration tasks
  • Acting as the primary point of contact for our support staff and clients, responsible for building rapport and engaging with and rostering our casual workforce
  • Providing advice and support for any immediate crisis situations and liaise with; clients, emergency services and support workers to achieve optimum outcomes

About you

As a passionate, reliable, and experienced Care Coordinator, you’ll have excellent verbal and written communication skills (including negotiation and problem-solving skills). Ideally, you’ll also have:

  • Customer service and rostering experience
  • Excellent interpersonal, negotiation and relationship management skills, coupled with a strong desire and willingness to learn
  • Ability to manage time effectively and meet established deadlines.
