Our mission and focus are to build individual’s capabilities to reach their goals through person-centred supports in NDIS Support Coordination, Plan Management and complementary services.

We are looking for a like-minded and passionate individual to join our growing team as a Support Coordinator. As a boutique business we encourage flexible working and aim to create a work environment where all employees are valued and can contribute freely.

We provide all necessary items for you to work from home as your main office base. To apply for this role, you must be able to have an appropriate area available to work from home and this area must be in line with our WHS policy. We use web-based systems to ensure team members can connect and work together quickly and easily. We get together face to face as a team fortnightly and ad-hoc throughout the week. We are a paperless organisation and ask all our employees to do their best to work in this manner.

As a Support Coordinator at LotUs Assist you will coordinate services and supports for NDIS participants with complex health and disability needs. The Support Coordinator will assist those who have Support Coordination funding as part of their NDIS plan, by providing coordination of supports to ensure their plan is implemented in a way that meets the person’s goals.

SKILLS & EXPERIENCE

Previous experience as a Support Coordinator will be highly regarded
Cert III Disability / Aged / Individual Support / Community Development OR working towards minimum Cert III and/or minimum1-year experience in the field of Community or Human Services
Experience working with people with Disability
Successful experience in coordinating responsive, individualised support to participants with complex needs and their families.
A sound working knowledge of relevant legislation, NDIS procedural guidelines and the full range of community services that may be required by participants
Current First Aid and CPR credentials
CRC, WWCC, Driver’s License, Right to Work in Australia
Sound administration and IT systems skills
