NDIS Support Coordinator
Part Time 20-30 hours per week, 12 month contract
As an NDIS Support Coordinator, you will develop and coordinate a wide range of supports which empower participants to live the life they choose. As a Support Coordinator you will responsible for;
· Providing guidance to deliver exceptional customer experiences by assisting the planning, delivery, monitoring and review of services.
· Providing people who have a disability with information, assistance, and support throughout each phase of NDIS.
· Work collaboratively to assist the Team Leader and peer Coordinators to research, coordinate and manage a range of varied levels of supports across multiple providers, to suit participant's needs.
To be successful in this role you will need:
· Qualifications in Community Services/Mental health/ Disability or studying towards a qualification or have a willingness to study towards a qualification.
· Demonstrated knowledge and experience of NDIS funded plans with minimum 2 years’ experience in a support coordination role or similar.
· Proficient skills in Microsoft Office and experience in the operation of corporate business systems.
· Proven experience building and maintaining professional relationships with customers, families and colleagues.
· Flexibility to adapt to changing work requirements.
· Ability to problem solve prior to escalating to the Team Leader.
· Demonstrated ability to apply initiative and work autonomously.
Mandatory Requirements to be successful in this role you will have:
· Working with children Blue Card - blue card is required prior to commencement.
· National Police Check - or be willing to obtain on commencement.
· Working with adults with disability card (yellow card) - or be willing to obtain prior to commencement.
· Valid drivers’ licence to operate a motor vehicle in Queensland.
