Established in 1912, Open Minds is a leading independent not-for-profit organisation and registered NDIS provider. Open Minds works to enhance mental health and wellbeing through tailored supports to people living with mental illness, disability, and employment.

Our Purpose is ‘enabling an independent and positive future’ for people with mental illness and disabilities.

The Case Manager delivers Mental Health psychosocial support services, on the Sunshine Coast and Gympie regions. NPS Case Managers work both autonomously and as part of a team to develop holistic plans to support people with mental illnesses. 

The NPS program is an integrated model that demonstrates the pillars of personal recovery, evidence- based service approaches, increasing community connectedness and building community capability for people with severe mental illness and their families.

The NPS case manager works proactively with people who experience severe MH conditions and could be facing impaired ability to function and stay safe in the community. This service may include enhancing social and emotional skills, building family connections, managing daily living needs, housing and financial requirements, vocational skills, physical wellbeing, and broader life skills. Assistance with NDIS applications may be required.

Nominated Term / Full-time - funded until 30/06/2021

Qualifications / Experience:

Education and qualifications

  • Professional degree or Cert IV (MH or related) and significant relevant experience working within the health, community or social service sector is essential.

Mandatory requirements

  • Hold and maintain a current Working with Children and Positive Disability Screening check relevant to the State or Territory you work in and in line with the relevant legislation.
  • Open C class driver’s licence.

Essential skills, knowledge, and experience

  • Previous experience in working in a team in mental health services and/or disability setting with experience in working in Complex Case Management & crisis response is essential.
  • Experienced & passionate about working in a team committed to making a positive change in the lives of people with severe mental illness.
  • Capacity to work independently and collaboratively within a team environment including being accountable for the effective delivery of NPS services and above average organisational skills.
  • Highly developed communication (including written), negotiation & conflict resolution skills. Excellent interpersonal skills, and the ability to engage with both the people with severe mental illness (and their families) and the service providers and community that surround them.
  • Demonstrated skills in developing individual support plans and risk management plans

Desired skills, knowledge, and experience

  • Understanding of Disability Services Act (Qld 2006), Mental Health Act (2016) and Human Services Quality Framework.
  • Demonstrated ability to professionally engage, liaise and network with stakeholders, build community relationships, and facilitate engagement with other service providers
  • Willingness to support complex clients with a range of mental health issues.
