JOB DESCRIPTION

E4 Recruitment are looking for a passionate and dedicated Remote Area Nurse who is willing to work with the community to engage and educate individuals regarding Chronic Diseases. 


Core aspects of your role will include:
  • Chronic Disease Program 
  • Provide holistic care, support and maintenance to the community 
  • Support clinics 
  • Complete all administrative follow up requirements associated with chronic disease clients
  • Prioritise and actively follow up on chronic disease clients
  • Provide advocacy and support for chronic disease self-management

Must haves: 
  • AHPRA registration 
  • Minimum of 3 yrs nursing experience - working in a Chronic Disease program
  • Knowledge of social determinants of health issues affecting Aboriginal people
  • Primary Health Care knowledge
  • WWCC & Police Check 
  • Current Manual NT drivers’ licence
