Lifestyle Coordinator

Bangka Strait - Daw Park

About the Role:
We have a fantastic opportunity to join the Lifestyle team at Bangka Strait - Daw Park on a 6-month Contract working 52 hours per fortnight.

The Lifestyle Coordinator, working alongside allied health teams will facilitate individual personalised lifestyle programs and activities that have a strong restorative focus to support clients and enhance their wellbeing and independence.

Duties:
Develop strategies, activities and programs which engage and empower people
Facilitate individual and group activities as per strength based approach
Support the person to maintain and/or improve physical, cognitive, emotional and social health through lifestyle activities which retain and develop abilities, skills and interests and enhance their wellbeing
Provide support to enhance an individual's life skills
Maintain documentation via the computerised database
Lifestyle care plan review
Liaise with internal and external stakeholders including the resident's family members, carers and allied health professionals to support resident

Skills & Experience:
Experience in implementing lifestyle and leisure activities
Skills to engage individuals and enhance & develop abilities, skills and interests
Skills to coordinate small to large groups, staff and volunteers
Excellent interpersonal, communication and problem solving skills
Qualification in Leisure and Health or equivalent
Demonstrated experience in delivering lifestyle techniques to people living with memory loss
