Social Work Services
The Social Work team across Monash Health is responsible for the delivery of a broad range of Social Work services for clients across the lifespan in the inpatient and outpatient setting.
About the Role
The Social Work Department is looking for an experienced Grade 2 social worker to join the social work team across Moorabbin Hospital and Monash Medical Centre. This is a part-time (0.8) permanent role and previous oncology and hospital experience is essential. This is an interesting and dynamic role where you will have the opportunity to work in the chemotherapy day unit, oncology and surgical acute wards, at Moorabbin Hospital two days per week and the other two days on the Gastro / General Medicine acute wards at Monash Medical Centre. This clinical role provides an opportunity to showcase your skills in working with people from a range of diverse backgrounds as well as completing complex psycho-social assessments and development of patient centred care plans. It is a requirement of this role to participate in the Emergency Department and on-call rosters.
Deliver and drive Monash Health strategic intent to be relentless in our pursuit of excellence across Social Work and Allied Health.
Work closely with the Social Work Manager to provide streamlined services across all campuses and lead designated services/programs/portfolios.
Travel is required across Monash Health sites.
About You
You will be a dynamic, innovative and flexible clinical practitioner with a good understanding of the treatment and psycho-social issues facing oncology patients and their families with a sound understanding of oncology pathways and referrals networks. You will also have an understanding of the aged care sector, chronic health conditions and related services. You will be able to demonstrate skills in psycho-social assessments, problem solving, communication and hospital discharge planning. You ensure that high quality Social Work services that place the client at the centre are provided. Education, quality improvement and evidence based practices will be your key drivers. Due to your proven clinical experience, you will bring knowledge of healthcare reforms and initiatives and challenges that public healthcare is facing now and in the future.
What you need
You must be eligible for AASW membership including continuing professional education
Proven experience in providing social work in acute and subacute settings.
Knowledge and experience working closely with inter-professional teams.
Current Driver’s Licence.
