- Indicative Total Remuneration: $79,954 - $83,378 p.a - Temp F/T (up to 30 June 2022) - More than one position - OPS4
SA Health is leading significant reform designed to improve the health of all South Australians and meet future challenges, in accordance with the National Aboriginal and Torres Strait Islander Health Plan and Close the Gap initiatives.
The Aboriginal Health Practitioner will be responsible for working collaboratively with Aboriginal consumers, general practitioners and practice staff to develop comprehensive care plans to improve the health of Aboriginal people with complex needs and assist Medical Officers through care coordination and access to other services while ensuring a culturally safe service is provided.
The Aboriginal Health Practitioner will work within a multidisciplinary team and is integral to ensuring cultural integrity, high standards of clinical care and client follow-up encompassing seamless management of Aboriginal people’s care plans within the community.
This role will contribute to South Australia’s Strategic Plan target 2.5 on Aboriginal life expectancy, as well as the Council of Australian Governments (COAG) National Partnership Agreement on Closing the Gap in Indigenous Health Outcomes health related targets.
To be eligible to apply you must have Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care (Practice) or equivalent, and be prepared to apply for registration with the Aboriginal and Torres Strait Islander Health Practice Board of Australia (part of AHPRA) and acquire registration on completion of the training within an agreed time frame.
Appointment will be subject to a satisfactory Criminal History Screening/Check and specific immunisation requirements.
