The Role

Client Services Coordinators are responsible for ensuring that high quality person-centered services and supports are provided to clients through implementation of individual support plans, effective management of support workers, and ongoing liaison with customers and contract managers. This role will be a floating role, covering the Far North / Eyre Peninsula. This is a part time role (hours to be negotiated).

Key Responsibilities

  • Act as primary contact for clients, funding bodies and Support Workers in designated region
  • Monitor and ensure adherence to Service Agreements and ensure all contractual documentation is accurate and up to date
  • Ensure clients’ requirements are matched with appropriate supports and initiate recruitment with HR if and when required
  • Assessment, mitigation and reporting of client and WHS risks
  • Management of escalated care
  • Manage team of Support Workers including identifying training and development needs

Key Requirements

  • Minimum of Diploma in Health / Community Services
  • Certificate in Training or HR (desirable)
  • Minimum 2 years experience in a similar role / care coordination
  • Work within the person-centred care ethos including demonstrating experience in client advocacy
  • Demonstrated experience in people management / leadership
  • Ability to manage autonomous and mobile teams
