About the facility
The Hospital Admission Risk Program (HARP) is a community-based program employing approximately 30 staff and can include nurses, physiotherapists, social workers, occupational therapists, a psychologist, dietitians, pharmacists and administration staff.
HARP aims to decrease hospital demand for people with chronic health and/or complex psychosocial needs. This is achieved through the provision of comprehensive assessment, care coordination and facilitating access to timely, specialist care in the community setting. Client support often occurs in people's homes or an agreed community setting.
About the role
As a HARP Team Leader, you will provide clinical leadership and support to a group of HARP Community Clinicians, in addition to line management, coordinating day to day work load, leave management, professional development reviews, accountability sessions and participate in and lead quality improvement initiatives.
The clinical component of the role is provision of direct client care to HARP clients.
