Remuneration: $32.46 - $52.35 per hour - dependent on qualifications

Dementia Support Worker
Inverell Community Health Service

About the role:
As the Dementia Support Worker you will provide a high quality dementia support service to patients and carers to ensure the provision of optimal health outcomes for patients of the service. You will act as a clinical resource and leader within Inverell Community Health in the provision of support services for people with dementia or at risk from dementia and their carers. You will also work collaboratively with appropriate health and external stakeholders to support early and accurate diagnosis of dementia and promote best practice dementia care in this rewarding role.

In this role you will provide a service to both Bingara and Warialda and work closely with a Dementia Support Worker in Armidale and the Clinical Nurse Consultant Cognition and Memory Service. You will be supported in your development with online training and attendance at Dementia Partnership meetings.

You are an enthusiastic and self-motivated individual with the ability to work well both independently and collaboratively as part of a dynamic multidisciplinary team. You have excellent communication skills, with a strong focus on time management, work ethic and the ability to work with people from various diverse backgrounds and provide individualised care.
