JOB DESCRIPTION

We are seeking an experienced Specialist Support Coordinator to join Adelaide's newest disability services provider to deliver services to people living with a disability. 

We are a locally owned and operated organisation building a therapy team to provide high quality, client-centred services. 

Our founders have over 30 years combined experience in disability and not-for-profit leadership and are focused on building a values-based organisation with a person-centred service approach. 

This role is in response to strong demand for our services and will be responsible for the following: 

  • Providing specialist support coordination to address high level risk and assist the participant to connect with supports and build resilience 
  • Actively monitor and develop participant’s NDIS plans and report on progress and outcomes 
  • Develop intervention plans for implementation by disability and mental health support workers
  • Use a strength-based and person-centred approach to build skills capacity of participants, ensuring their goals and choice drive the process of engaging support

To be successful in your application you will 

  • Be tertiary qualified in Psychology, Social Work, or other allied health disciplines
  • Possess knowledge and experience of NDIS funded plans with a minimum two years' experience in a specialist support coordinator role
  • Have exceptional communication skills, both verbal and written, particularly in advocacy, mediation, negotiation and conflict resolution
  • Be able to build rapport and maintain strong working relationships with clients and their families 
  • Have the ability to work as part of a team as well as independently
  • Be a team player and contribute to a positive workplace culture
  • Hold current NDIS Worker Check/National Police/DCSI Check
