Employment Type: Temporary Part Time (57 weeks)
Position Classification: Level 3 Social Work, Physiotherapy, Speech Pathology, Dietetics, Counselling, Occupational Therapy, Genetics Counselling, Psychology, Registered Nurse or Midwife
Remuneration: $32.47 - $52.36 per hour, dependent on qualifiactions
Hours Per Week: up to 24

The Out-of-Home Care Health Case Manager is responsible for the coordination of health care by improving access to health services with the goal of improving health outcomes for children and young people in statutory Out of Home Care. Aiming to reduce the health disadvantage experienced by children and young persons in statutory care.

Selection criteria


  1. Relevant tertiary qualification and registration with the relevant board or eligibility for membership of the relevant association, in one of the following disciplines: Social Work, Physiotherapy, Speech Pathology, Dietetics, Psychology, Counselling, Occupational Therapy, Genetics Counselling, Registered Nursing or Registered Midwifery(to be appointed as a CNS2 the Registered Nurse (Division 1) must have a postgraduate qualification in the this area or related field and at least 3 years’ experience working in the area or related area).
  2. Demonstrated knowledge and understanding of case management principles, Child Protection Legislation, Information Exchange and NSW Health GL2013_10 Health Assessment of Children and Young People in Out- of-Home Care (Clinical Practice Guidelines).
  3. Proven commitment to improving health outcomes for at-risk populations.
  4. Demonstrated knowledge of existing health referral pathways for children living in NNSWLHD/NSW.
  5. Highly developed time management skills with interpersonal, written and verbal communication skills and demonstrated capacity to work in partnership with others.
  6. Demonstrated knowledge, experience and proficiency in using computer programs (eg Microsoft Excel, MS Word, Intranet/ Internet and Outlook email and calendar functions), electronic medical records and medical record management both paper and electronic.
  7. Valid unrestricted drivers licence for use in NSW/Australia and willingness to travel in the course of employment.
