ConnectAbility is offering an exciting opportunity for a dedicated and passionate Support Coordinator/Psychosocial Recovery Coach to join our team on a 12 month fixed term full-time basis. We are looking for individuals who are passionate about empowering and enhancing the lives of those living with a disability and/or psychosocial illness.

About the role

This pivotal role will see you liaise with a range of internal and external stakeholders to allow each customer to maximise their NDIS plan and achieve their individual goals, build capacity and increase resilience for each of our customers. The ideal candidate will also bring an open and creative mindset to the position. In this role you will:

  • Manage a portfolio of customers, completing agreed activities within the hours and funded budget articulated in customers’ NDIS Plan
  • Assist customers to connect to, engage and build capacity with, informal and mainstream funded supports and services as articulated in their NDIS Plan and which assist them achieve their goals
  • Developing a recovery-enabling relationship with customers with psychosocial disability
  • Build and sustain relationships with external service providers and mainstream supports for the purpose of maximising opportunity for customers
  • Build capacity in customers and/or their family to become as self-sufficient as possible about the NDIS, coordinating their supports and achieving their goals

You will be interacting daily with formal supports (community/health providers and disability service providers), informal supports (friends/family members) and your customer directly. In addition to your role as coach and advocate you will also be tracking plan budgets, writing progress reports, liaising with the NDIA and generally ensuring things run to plan.

This position is on a full time basis, however part time will be considered for the right applicant.

About you

  • Tertiary qualifications or experience in Disability, Mental Health, Social Work, or related behavioural science and/or demonstrated experience in related work role
  • Experience in working with people with disabilities and their families/carers
  • Experience working with people with a disability and psychosocial disabilities in a support coordination role
  • Experience as a Recovery Coach, Support Coordinator, Local Area Coordinator and/or NDIS Planner desirable
  • Knowledge of the NDIS Act 2013, Disability Act 2006 and other relevant legislation
  • Highly developed written, verbal, and service collaboration skills including the ability to deal openly and effectively with people with multiple and complex needs
  • A demonstrated robustness, resilience and level of maturity that supports independent practice 
  • Capacity to monitor service provision with budgetary requirements and contribute to the budget development process
  • Well developed interpersonal and negotiation skills 
  • High level of problem solving skills with the ability to work efficiently under pressure and meet deadlines
  • Excellent time management and organisational skills with the ability to handle multiple conflicting priorities.
  • Demonstrated experience with facilitating multi-disciplinary coordinated planning where multiple providers work together to wrap-around services for people with disabilities and develop collaborative relationships and networks between agencies.
  • Ability to use Microsoft Office Suite and customer information management systems.
  • Current Drivers’ License and the ability to undertake travel as required
  • Current NSW Working with Children Check
  • Current NDIS Worker Screening Check (NDISWC)
  • NDIS Worker Orientation Module Completion Certificate
  • NSW Department of Health COVID-19 Infection Control certificate
