What you'll be doing

The Care Facilitator will provide critical links between primary, community and hospital services for patients registered for Western Sydney Integrated Care Program (WSICP), a service developed as part of the NSW Integrated Care Strategy.
In particular, facilitate the provision of coordinated and timely clinical care for people with severe chronic disease conditions such as diabetes, COPD and CHF, who are at high risk of hospitalisation, across health service sectors including primary, acute, secondary and community services.
The position will work closely with Hospital Specialist teams and targeted General Practices to support patient enrolment, care planning, care navigation, care transfer, health outcome monitoring and patient education.
