Service Delivery Coordinator – Home Care Services

We currently have an exciting opportunity for an enthusiastic and professional Service Delivery Coordinator to join our Home and Community Services team in Regional New South Wales. This is a one year contract with provision for extension.

The role can be based in our Tamworth or Inverell office and may require you to travel occasionally to our regional offices and communities we serve.

The Service Delivery Coordinator is responsible for ensuring our clients receiving high quality care services to enable them to continue living the life they love.

The successful candidate must have:

  • Current Influenza vaccination
  • Minimum Cert IV in Case Coordination or equivalent
  • Cert III Individual Support
  • First Aid certificate and CPR
  • Minimum of 3 years experience in a similar role within Human Services
  • Full drivers licence

Responsibilities include:

  • All leave management for staff members
  • Scheduling issues for clients by liaising with team members / coordinators / CCC
  • Recruitment
  • Onboarding new staff
  • Oversee external stakeholders engagement and relationship building with referrers
  • Admin / staff meetings
  • Initial contact with staff members for any issues
  • Staff incidents
  • Assist with Performance Development Reviews
  • Oversee Roster Team
