THE OPPORTUNITY

The Intake and Service Coordinator is the designated point of contact for all prospective participants, their families and/or representatives providing a warm and responsive service to move participants through the pre-engagement process to service delivery, coordinate the delivery of support services delivered under the NDIS, provide operational support to the Operations Manager, and support the day-to-day delivery of services to NDIS participants.

The position is responsible for the implementation of a coordinated intake process that identifies participant support needs including co-designing supports, development of service quotes and client agreements, and the collation of client specific data. The Intake and Service Coordinator instils confidence in participants and key stakeholders to choose Sunshine Kids - Musha Vana as their provider of choice.

KEY RESPONSIBILITIES

Business Development

  • Promote the unique value proposition of Sunshine Kids - Musha Vana
  • Promote the dignity and positive image of participants within their communities, maximising the opportunities for participants to join in and contribute to the communities in which they live
  • Link with participants and their families, and support coordinators (as participant representatives) to facilitate access to community resources
  • Develop care plans that are consistent with NDIS plans to ensure achievement of key outcomes and goals and present to participants (or their representative) for approval
  • Develop appropriate quotes that meet the needs of participants and the organisation
  • Determine participants' eligibility for the organisation's services and refer prospective participants to other more relevant services where appropriate
  • Obtain accurate information for participant case files and organisational records within a person-centred active support framework

Service Delivery

  • Conduct preliminary support assessments as needed and perform initial and ongoing risk assessments with participants
  • Ensure effective and professional communication with the Operations Manager and provide monthly service reports
  • Liaise with the team around actioning new participant services to ensure positive intake experience and timely commencement of service delivery
  • Liaise with internal and external stakeholders to ensure successful facilitation of NDIS plans and support of participant outcomes
  • Lead the support team to achieve exceptional service outcomes

ABOUT YOU

  • Qualification in Disability or a relevant field (highly advantageous)
  • Demonstrated broad experience in service delivery and quality systems
  • Sound knowledge of the NDIS Practice Standards and Compliance
  • Understanding of the NDIS Quality and Safeguards Commission and use of the NDIS portal
  • A passion for leading, empowering, and supporting a close-knit and results-oriented team
  • A client-focussed approach
  • Excellent and adaptive communication skills
  • Excellent relationship building and management skills
  • Experience in risk assessment, facility compliance, and quality assurance
  • High levels of I.T. literacy in using a range of software packages including proficiency in the use of Microsoft programs such as Word and Excel, and able to learn new systems with ease.

Required prior to employment:

  • NDIS Worker Screening Clearance
  • WWC Check
  • Police Check
  • Driver Licence and own reliable transport
  • First Aid and CPR Certificate
